Annandale

  • Girls ages 12 to 18

  • Mental health treatment; mild chemical dependency

  • Residential placement is also offered to females in need of Protective Services (CHIPS).


Cokato

Referral and Intake Packets 

  • The residential intake packet does not need to be completed to make a referral. For a referral, we are looking for a DA, psychological/ psychiatric evaluation(s), progress/discharge reports, IEP, and any other information that is pertinent for screening.
