MnCHOICES is a free, in-person assessment to determine eligible services for individuals with disabilities.

After the assessment, you will receive a summary of the assessment along with information about the programs or services that you or your child qualifies for. From there, you can choose providers and develop a care plan. Once your plan is approved and all the paperwork is completed, you/ your child can start getting services.
