Children's Mental Health Case Management

• A professional referral is required. A mental health professional must submit the referral form with a diagnostic assessment completed in the last 180 days 

• Developing care plans and crisis plans.

• Providing information about and referrals to community resources.

• Creating a supportive team of family, professionals and community members.

• Assisting parents with their child’s mental health needs.

• Providing access to other support services.

Adult Mental Health Case Management

• Professional referrals must include a diagnostic assessment and statement of need

• Self referrals should visit the Mental Health Center at 1919 University or call 651-266-7890 to discuss a possible referral to targeted case management services.

• Case managers work with the client to develop a support plan and make referrals. Case managers do not provide direct services.

• Local agencies provide Adult Rehabilitative Mental Health Services. ARMHS is a direct-service program that teaches, coaches, and supports clients in developing life skills impaired by their mental illness. A client can have a Case Manager and also an ARMHS worker.
